Confidentiality
The therapist is obliged to act in accordance with legal regulations. The rights and obligations of the patient and the therapist are laid down, inter alia, in the Medical Treatment Agreement Act (WGBO) and the Care Quality, Complaints and Disputes Act (Wkkgz).
As a therapist, I keep a patient file. The patient has the right to inspect their own file. This file is kept for a total of 20 years in accordance with the legal retention period.
I have a duty of confidentiality regarding information provided by the patient, which is stored in the file. At the patient’s request, prior, during or after the treatment period, information may be obtained and/or provided from/to an appointed therapist, physician and/or medical specialist for which the patient must specifically consent (your therapist never provides data to third parties without your express consent).
Appointments treatments/consultations
As a therapist, I am obliged to provide information regarding treatment to the patient at all stages of treatment. The patient undertakes to provide relevant information to me through the history interview.
The therapy plan consists of a number of consultations, in each of which a conversation takes place both at the beginning and at the end of the consultation. At the end of each consultation it is also decided whether the therapy will continue.
The therapist may not perform therapeutic acts without the patient’s consent. For a minor patient and/or a patient under guardianship/supervision, parents and/or guardians/supervisors must give permission.
The therapist agrees to correctly refer the patient to a colleague (therapist, physician and/or other medical provider), if the patient’s own treatment is not appropriate and/or adequate.
Cost
The costs of treatment are listed on the website of Praktijk voor Mesologie en Orthomoleculaire therapie Dafna Suskind. This rate applies both to the first consultation including intake interview, as well as per session for subsequent consultations.
Individual dietary recommendations are included in the consultation and are provided in writing after the consultation or can be viewed on the website. Of the findings of the initial examination, patient will receive a written report by email if necessary.
There is a charge per report for preparing an additional written report for patient, or at patient’s request for a therapist, physician and/or medical specialist.
Future rate adjustments will be announced via the website (www.mesoloog.amsterdam) at least 3 months prior to rate changes.
Payment
The invoice is sent to patient by email. This allows patient to easily submit the application for (partial) reimbursement based on her/his additional health insurance to the health insurance company digitally. Payment is made in full and following the consultation by pin or cash. Payment by credit card is not possible. The method of payment of additional costs is by bank with a payment period of 14 days.
Cancellation
If unable to attend, patient will cancel at least 48 hours (two business days) in advance (either by phone notification, leaving a message on voicemail or sending a message via email). Appointments cancelled within 48 hours may be charged.
Accessibility
Practice for Mesology and Orthomolecular Therapy Dafna Suskind works by appointment only. If I am unable to make an appointment due to illness or unforeseen circumstances, I will notify you as soon as possible and contact you as soon as possible to schedule a replacement appointment. If I am unable to perform consultations myself for a period longer than four weeks due to illness or other reasons, a fellow therapist from the Integral Medical Center will be called in. The patient always has the choice whether or not to agree to this.
Liability
The practitioner’s advice is by its nature result-oriented without guaranteeing that result. The practitioner excludes any liability for damage or injury or illness arising from or related to the client’s following of advice given by the practitioner, unless there is intent or gross negligence on the part of the practitioner. On the understanding that only those damages for which the practitioner is insured are eligible for compensation.
The following limitations have to be taken into account; Not eligible for compensation in
is trading loss, loss of income and the like, whatever the cause. The therapist is never liable for damage caused by intent or gross negligence by third parties. If the insurer of the therapist does not pay out for whatever reason, the liability of the therapist shall always be limited to no more than twice the rate of 1 consultation. The practitioner is not responsible or liable for the consequences of incorrect information resulting from an unknown and/or concealed health issue.
Conditions precedent
Termination of treatment may be by mutual consent at any time. If the patient no longer appreciates continuation of the treatment or does not consider it necessary, he/she may unilaterally terminate it. The therapist can only unilaterally terminate the treatment with arguments and refer the patient, if necessary, to another therapist, doctor or other medical care provider.
